Before participating in any of RISE Training and Nutrition services, please read, sign, date and submit the forms below.

By entering into this personal training agreement or purchasing any other programs, services, and/or products we offer, you are agreeing to accept all parts of this disclaimer. Thus, if you do not agree to this disclaimer below, STOP now, and do not use our app, programs, services, or products.
You should consult your physician or other health care professional before starting this or any other nutrition or fitness program to determine if it is right for your needs.
We are not licensed medical care providers and do not represent that we have any expertise in diagnosing, examining, or treating medical conditions of any kind, or in determining the effect of any specific nutrition program or exercise on a medical condition. Any information we may provide is not intended to be a substitute for professional medical advice, diagnosis or treatment.
You should understand that when participating in any nutrition and exercise or exercise program, there is the possibility of physical injury. If you engage in this nutrition, exercise or exercise program, you agree that you do so at your own risk and are voluntarily participating in these activities. You assume all risk of injury to yourself, and agree to release us from any claims or causes of action, known or unknown, arising out of our services to you.
We make no guarantees concerning the level of success you may experience, and you accept the risk that results will differ for each individual. Any testimonials and examples provided are not necessarily typical results and are not intended to represent or guarantee that anyone will achieve the same or similar results.

RISE TRAINING and NUTRITION LLC Covid-19 Waiver

Rise Training and Nutrition LLC cannot prevent you from becoming exposed to, contracting, or spreading COVID-19 while utilizing RISE Training and Nutrition’s services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you choose to utilize RISE Training and Nutrition's services and/or enter onto RISE Training and Nutrition’s premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 in order to utilize RISE Training and Nutrition’s services and enter RISE Training’s premises. These services are of such value to me that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize RISE Training’s services and premises in person

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against RISE Training and Nutrition LLC and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing RISE Training and Nutrition LLC’s services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

CHOICE OF LAW: I understand and agree that the law of the State of New Jersey will apply to this contract.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE:
